I feel that prevention is the best policy. Many good free scanners are available including On-line scanners. A full-time active client/agent will prevent infection in the first place. Suggest that if you are going to invest dollars in solid anti-spyware you get this functionality. I use (and trust) A-Squared Anti-Malware. I have it set to update hourly and scan weekly. Now using almost 600,00 signatures to block (and remove) spyware.
It is also active against known Rootkits. Whichever of the applications you choose, select one with full-time active protection rather than just removal.
Note: A2 is compatible with most (if not all) major anti-virus programs.

It also best to educate yourself and the people who use your computer about what they are doing on the web and what they are installing on the computer. I have yet to run any spyware removal on any of my comptuers at home or my mothers. But my Uncle's son and his friends wreak havoc on my Uncle's computer all the time. I had to fix it twice last year because of stuff they install.
