Rockwell modem drivers. Can't find anywhere!!!

Im having trouble finding drivers for my modem, Please HELP!!?!?

i've looked everywhere for this darn thing and can't find the drivers on any site. any help would be great...

Here are all of the major numbers on the actual card...

This is also on there (not on the actual green part though)

and this sticker has the barcode along with this stuff under the barcode...
