Like in a previous post, it all depends on what e-mail program you are using. The best integrated ones I've seen only work with Outlook2000 or newer.

iHateSpam, Spam Inspect and Spambayes (which is free), all do a pretty good job and work right within Outlook. This way you don't need to keep checking mail with yet another program.

If you have to, then the Pro version of MailWasher is about your best choice, as noted in some of the other posts. You just need to keep remembering to use it before you send or receive mail.

I have a user name for my ISP and ISP that is not used any where and it has letter and numbers in it. I don't use it to sign up for anything and have only give out the address to a couple of people.
Before I had with my old ISP the name hewee. But that user name was used all over the place and I had signed up places with it before I knew what spam was. My ISP also spam me. Was Pacbell and then SBC. they said they never spam but they did. Later they became SBC/Yahoo and after that they changed mail servers over to yahoo. All the same type of spam you get over and over and over for years had stop and then I was getting all new types of spam from new places. Now it was not MailWasher that did that but my ISP because they were the spammers.
I have Comcast cable know and I made sure from the very start that I did not want to be on and list of news letters or get info from 3rd party. I mean you can get it and then opt out from getting it but there is something in there where even after you op out they can still sent you things. I forget just what it was but it was posted over at the comcast forum. Makes you mad but you got to read the find print because just seeing that they will let you opt out does not do any good if they gave your address out to others.
Once they have your address your in trouble so keep it safe.
I have hotmail setup so all spam gets deleted right away and had to setup filter to delete the spam from hotmail because they say there spam is not spam but info you may like. Then I got MyWay for web email that has been good. I do get some spam but block the address or ISP it comes from.

The good thing about spam inspector is that you do not have to load any other program to check your mail. After you install the program, it is now part of Outlook. I dislike having to load two programs to check my mail.

Personally, I wouldn't like one software messing around with the installation of other software. Too much close integration is troublesome sometimes. I would prefer the way mailwasher works. Separate from OE installation, no integration. So I can use it as and when I want and can easily stop using it without any worries.
